Why Bruising Happens
When a needle passes through the skin, it can nick tiny capillaries in the subcutaneous tissue. Blood leaks into the surrounding area, creating the characteristic blue-purple mark. This is a minor mechanical event — not an allergic reaction or sign of medication problems. Bruises typically resolve within 5-10 days.
7 Ways to Prevent Injection Bruising
1. Apply Gentle Pressure After Injection
After removing the needle, press a clean cotton ball or gauze over the site with gentle, steady pressure for 30-60 seconds. This compresses any nicked capillaries and allows clotting before blood spreads into tissue. Do not rub — rubbing disperses blood and makes bruises larger.
2. Ice the Area Before and After
Applying ice for 20-30 seconds before injection constricts blood vessels, reducing the chance of bleeding. Applying ice for 2-3 minutes after injection helps minimize any bleeding that does occur. This is particularly effective for patients on blood thinners.
3. Use the Thinnest Needle Available
Thinner needles (31-32G) cause less tissue trauma than thicker ones (29G). If you are experiencing frequent bruising with a 29G needle, switching to 31G may help. The trade-off is slightly slower medication draw from vials.
4. Avoid Visible Blood Vessels
Before injecting, examine the planned site for visible veins or capillaries under the skin. Move at least half an inch away from any visible blood vessel. Good lighting helps identify these areas.
5. Inject at the Correct Depth
GLP-1 medications should be injected into the subcutaneous fat layer. Injecting too deeply (into muscle) or too shallowly (into dermis) increases bruising. Use proper skin pinch technique and the appropriate needle length for your body composition.
6. Rotate Sites Consistently
Repeated injection in the same area makes tissue more prone to bruising over time due to micro-trauma accumulation. Proper rotation allows complete tissue recovery between injections.
7. Time Blood Thinners Appropriately
If you take aspirin or blood thinners, ask your provider about optimal timing relative to injection. Some patients find injecting in the morning before taking evening blood thinners reduces bruising. Never change medication timing without medical guidance.
Treating an Existing Bruise
First 24 Hours
- Apply ice wrapped in cloth for 10 minutes on, 10 minutes off
- Avoid heat, hot baths, or vigorous exercise
- Arnica gel or cream may help reduce discoloration
After 24 Hours
- Switch to warm compresses to promote blood reabsorption
- Gentle massage around (not directly on) the bruise
- Most bruises resolve within 7-10 days
